Here's a few that I've found (we needed one for our ISP here, too):

One or Zero - http://helpdesk.oneorzero.com/ - For the majority, does what
you'll need.
IRM - http://irm.schoenefeld.org/ - Even has some site monitoring aka Big
Brother..
Helpdesk Ticket System - http://www.tekrat.com/projects/hts/index.php - Not
pretty at all, but it works.  Even has NT authentication for usernames

There's my take. :)
