I've seen similar behavior with the User Profiles dialog having
multiple profiles and "don't ask at startup" unchecked.  I think that
the first thing Firefox tries to do is check to see if there is
another Firefox window in the user's display environment and if so, it
just opens a new window from that instance rather than making a
totally new window.

Have you tried asking on the mozillazine forums?  http://forums.mozillazine.org/

They are usually a very helpful bunch.
