That link did have an important tip to show what the pcmcia card ident is:

$ cardctl ident
Socket 1:
  product info: "Network Everywhere", "Fast Ethernet 10/100 PC Card", 
"3.0", "AX88190"
  manfid: 0x0149, 0xc1ab
  function: 6 (network)

This matches the entry in my /etc/pcmcia/config file:

   1041 card "LinkSys Network Everywhere NP100"
   1042   version "Network Everywhere", "Fast Ethernet 10/100 PC Card"
   1043   #manfid: 0x0149, 0xc1ab
   1044   #bind "pcnet_cs"
   1045   bind "axnet_cs"

kubuntu shipped with the manfid commented [line 1043].  I tried after 
commenting it and no dice.

I'll have to do more research and report back.
