[clue-tech] Web form contact list software?

Dave,

As I was going to sleep the other night, I realized that I had 
fat-fingered my message.  I should have typed "phpMyadmin".

I have used it with Apache as localhost to administer some databases.  I 
have used it with both Linux and (shudder!) Windows XP, primarily by 
installing xampplite.
