I've run tune2fs to set -r0m0i0 etc. on mounted filesystems (in spite of the tune2fs warning) lots of times and so far it hasn't broken anything. Generally I did those changes on the mounted filesystem because I didn't want to reboot, it was the root filesystem, or some other reason like that. I think that tune2fs can do some things that will break a mounted filesystem but I haven't had any trouble changing the mountcount etc. on mounted filesystems.
