Here's an example proposal (Star / Open Office format).  It was for a web site 
this past summer.  The job did not come through because of costs, but the 
client liked the proposal.  I will show a proposal to the client and discuss 
any changes.  I then append a signature sheet to the proposal once the 
changes are OK with everyone.

Also, with regard to a specific hourly rate, I have worked anywhere between 
$40 and $90 per hour.  My current minimum is $50 / hour for SA and static web 
page design and $65 for programming.  I only did this part time for the past 
4 years, so I could hold firm to my minimums.

Like I said before, I never list an hourly rate for the project, but I use my 
absolute minimum * expected hours to determine the fee for each major 
component.  I also list optional components.  I will list hourly rate for 
future changes, support, etc.  I have found that owners want the world, but 
when you put a price on it, they will drop some features to save money.  This 
is better for you since it won't drop your hourly rate - better for him if 
he's on a budget for the project.
