Based on your examples you also want composability (server A contains file systems 1-3, oh wait,
file system 2 just moved to server B).
You want a CMDB. ITIL has a lot to say about that. I don't know of any Free ones. There are
certainly enterprise CMDBs you can buy (and should avoid, like all "enterprise" software).
A wiki with good templates will do what you want, in a way that doesn't force structure. Then you
have to figure out how to get it updated easily, and how to automate checking that it's accurate.
So you don't have to start with a DB. Start with your monitoring system, teach it to know what you
have, then teach it to verify your related data.
Probably the USENIX/LISA people would be good to ask.
