I'd recommend 4dv.net. My company has been with them for several years 
without any problems and I've set up many a client, friend and family 
member with them and have never had a problem. They are fast to respond 
to any issue you may have (and I've had very little). The only problems 
I've had (and it has nothing to do with 4dv) are with those wonderful 
Actiontek routers that Qwest gives out. Anybody have a Cisco they want 
to give up for cheap? ;-)
