In my experience (about 10 months), forethought.net meets all 4 of those
criteria; a basic DSL account with them costs $20/month. Also, they have
the most competent and courteous staff of any ISP I've dealt with.

Actually, #2 probably has more to do with your DSL provider (Qwest or
...) than the ISP.
